WESTPORT, CT (Reuters Health) – In prepubertal girls with premature adrenarche and/or obesity, the fasting glucose-to-insulin ratio (FGIR) and the quantitative insulin sensitivity check index (QUICKI) provide an accurate picture of insulin sensitivity, according to the findings of a study published in the June issue of The Journal of Clinical Endocrinology and Metabolism.

Dr. Sharon E. Oberfield and a team from Columbia University in New York note that "insulin resistance is a strong predictor of the development of type 2 diabetes mellitus and cardiovascular disease. Girls with premature adrenarche or obesity may be at increased risk for the development of insulin resistance."

In a comparative study involving 25 such girls, insulin sensitivity based on the FGIR and QUICKI correlated strongly with two measures of insulin sensitivity based on the more involved oral glucose tolerance test (OGTT). These two measures were SiM, an adjusted mean measure of insulin sensitivity based on fasting and 2-hour, post-glucose-load insulin sensitivity measures; ISI(comp), the composite whole body insulin sensitivity index.

"In contrast to the multiple blood samples, length of time, and personnel required for the OGTT," the team notes, FGIR and QUICKI "can be obtained easily from a single fasting blood sample."

"An FGIR less than 7 in young girls with premature adrenarche or obesity may be helpful in the early identification of children at risk for complications of insulin resistance," they conclude.

The team also reports that girls characterized as insulin-resistant had increased body mass index and triglyceride levels compared with girls who were insulin-sensitive. Insulin-resistant subjects were three times more likely to have triglycerides above the 95th percentile compared with the norm.
