An official British watchdog said on Thursday that thousands of drug side effects go unreported and that when safety messages are issued they do not always reach those who need them.
The National Audit Office (NAO) said that 19,000 reports of suspected adverse reactions, more than half of them serious and 3% fatal, were filed by UK doctors last year.
