Early treatment of Kawasaki disease results in less coronary ectasia in children at one year after disease onset.
Early treatment is not associated with quicker resolution of fever or increased number of treatment failures.
It is not associated with increased need for adjunctive therapy, length of hospitalisation or development of coronary artery lesions either, Canadian researchers say.
“In children with fever and classical clinical and laboratory findings of Kawasaki disease, treatment with intravenous immunoglobin (IVIG) on or before five days of fever resulted in better coronary outcomes,” they report.
They also found the duration of clinical symptoms decreased. Investigators from the Hospital for Sick Children at the University of Toronto in Ontario studied 178 children diagnosed with Kawasaki disease between 1987 and 1999.
Participants had fever and at least four or five clinical disease criteria. Researchers matched 89 patients who received IVIG at day five or earlier with patients diagnosed within four weeks and given IVIG at days six to nine of fever (controls).
They compiled information from detailed chart reviews.
Data included demographics, clinical features, fever duration, investigation, disease course and therapy response.
