Investigators have developed a method to incorporate endobronchial biopsy safely into the evaluation of children with difficult respiratory problems.
Researchers at the Departments of Pediatric Pulmonology and Anesthesia Baystate Medical Center, in Springfield, Massachusetts, United States, reported this new method here Sunday at ATS 2002, the American Thoracic Society’s 98th International Conference in Atlanta, Georgia.
A major limitation in pediatric bronchial biopsy until now has been a concern over the safety of the procedure. “The most important thing we found is that this procedure — obtaining bronchial biopsies in children, even very young children — is exceptionally safe, said Paul S. Salva, MD, PhD. “Applying this information, I believe we now can go ahead and obtain these specimens and to help sort out the different disease states.”
In their study, the researchers examined the procedure in 120 children, ages 2.5 to 16 years, who had chronic respiratory symptoms. All patients were studied under general anesthesia in an outpatient setting.
For the procedure, the researchers used a flexible bronchoscope (4.9 mm OD) to obtain, via laryngeal mask airway, bronchoalveolar lavage and a minimum of three biopsies. Children younger than four years were given one dose of antibiotic prior to undergoing the procedure.
