Although intraoperative frozen section analysis has acceptable sensitivity and positive predictive value for diagnosing infection in total knee arthroplasty, it is not accurate enough for diagnosing infection in revision total hip arthroplasty.
Daxes M. Banit, MD, at the University of Kentucky Medical Center in Lexington, United States, and colleagues conducted a study to compare the sensitivity, specificity and predictive values of frozen sections vs. cultures obtained at the time the revision total arthroplasty was performed.
The prospective study involved 92 men and 28 women who underwent 121 joint replacements. For each, a positive frozen section with more than 10 polymorphonuclear leukocytes per high power field was compared with intraoperative cultures.
