Although intravenous tissue plasminogen activator (tPA) is the only thrombolytic therapy approved by the Food and Drug Administration, the arterial route may produce better outcomes in some instances, according to an analysis presented here January 30th at the Society of Critical Care Medicine’s 32nd Critical Care Congress.
Dr. Michael Torbey of the NeuroCritical Care Division at Johns Hopkins University in Baltimore, Maryland, presented results of a literature search and meta analysis of reported studies, which included only prospective or randomised controlled trials.
Using a Monte Carlo simulation, the researchers showed that the best outcome was with combined intravenous and intra-arterial administration of thrombolytic agents. Only prospective trials or large randomised studies were part of the analysis.
