Monoclonal Anti-Granulocyte Antibody Imaging Diagnoses Diabetic Pedal Osteomyelitis

Monoclonal anti-granulocyte antibody imaging compares well with in vitro labelled leukocyte imaging in diagnosing pedal osteomyelitis in diabetic patients, American research finds. The 1-hour antibody imaging is also significantly more specific than 3-phase bone imaging, say investigators from the Long Island Jewish Medical Centre in New Hyde Park, New York; Tri-City Medical Centre in Ocean City, California and Sutter Roseville Medical centre in Roseville, California.

They evaluated technetium-99m-labelled monoclonal anti-granulocyte antibody in comparison with indium-111-labelled leukocyte and 3-phase bone imaging in 25 diabetic patients who had pedal ulcers. The aim was to find a safe, fast and accurate in vivo method of labelling the white cells that diagnose diabetic pedal osteomyelitis. Twenty two participants had pedal ulcers in the forefoot, and three had ulcers in the mid-foot. All underwent 1-hour antibody, 24-hour labelled leukocyte and 3-phase bone imaging.
