Patients with coagulopathy have higher complication rates after treatment for stones in the urinary tract, even if the coagulopathy is controlled prior to the intervention. These patients also, appear to be better candidates for endoscopic procedures rather than extracorporeal shock wave lithotripsy (ESWL). H. C. Klingler and colleagues from the department of urology, University of Vienna, Austria, retrospectively evaluated various treatment protocols for upper urinary tract stones in patients with clotting disorders.
During the six-year evaluation period, 6,827 stone interventions were performed in 5,739 patients. Among these patients, 35 had systemic clotting disorders or were anticoagulated. The patients underwent 76 interventions, consisting of ESWL, uteroscopy (URS), percutaneous nephrolithotomy (PNL), ureteric stenting or percutaneous nephrostomy. The clotting disorders were all corrected by specific therapy prior to any intervention.
