Improper dosing and other medication mishaps continue to pose serious health risks for US hospital patients, particularly children and individuals treated in the emergency room, a new study finds. Drug errors are less likely to be intercepted before reaching kids, according to the US Pharmacopeia (USP), a nonprofit outfit that maintains the nation’s largest medical errors reporting database.
Dosing mishaps, which topped the list of errors affecting children 16 and under, appear to be the result of various factors, ranging from misplaced decimal points to harried nurses’ failure to check calculations.
