There have been three major randomised trials of surgery versus angioplasty with stentimplantation: ARTS, ERACI II and, now, the SOS Trial. Although there were large differences between studies in patients’ presentation, delay to treatment, surgical outcome, and stent type used, the results at long-term follow-up were consistent.
There were differences in late mortality. CABG patients had more symptomatic relief, and PCI patients had a higher need for repeat revascularization. Compared with balloon angioplasty, the need for repeat revascularisation has dramatically decreased for PCI patients. In BARI, 50% of angioplasty patients needed repeat revascularization versus 21% at one year in ARTS, 18% at one year in ERACI II, and 21% at 2 years in SOS.
