Refractory Pemphigus Responds to IV Immunoglobulin

Lesions of pemphigus vulgaris unresponsive to high-dose corticosteroid therapy apparently do respond to treatment with IV immunoglobulin and cyclophosphamide, according to a report in the Journal of the American Academy of Dermatology for September.

 A series of six patients with severe active disease had failed treatment with prednisone in doses ranging from 60 to 280 mg/day administered for at least 2 months. Dr. Jean-Claude Bystryn and associates at the New York University School of Medicine, New York, initiated treatment with human immunoglobulin 5%, 400 mg/kg daily for 5 days, plus cyclophosphamide 100 to 150 mg/day.
