Steroids Reduce Septic Shock Deaths in Adrenal Insufficiency

A seven-day treatment of low-dose steroids significantly reduced the risk of death from septic shock, but only in those patients with adrenal insufficiency, according to the results of a multicenter French study reported in the Aug. 21 issue of The Journal of the American Medical Association. The investigators recommend treating all catecholamine-dependent septic shock patients with hydrocortisone and fludrocortisone, but discontinuing this treatment if they respond appropriately to corticotropin.

“In our trial, a seven-day treatment with low doses of hydrocortisone and fludrocortisone significantly reduced the risk of death in patients with septic shock and relative adrenal insufficiency without increasing adverse events,” write Djillali Annane, MD, PhD, from the University of Paris in France, and colleagues.
