Prosthetic repair of umbilical hernia in adults associated with low recurrence rate

LONDON (Reuters Health) – Prosthetic repair using polypropylene mesh, already the standard for repair of inguinal hernia, should also become the standard for treating umbilical hernia in adults, Spanish researchers suggest in a report in the October issue of the British Journal of Surgery.

Dr. A. Arroyo and colleagues from University Hospital of Elche randomly assigned 200 men and women with umbilical hernia to herniorrhaphy (primary suture) or hernioplasty (mesh or plug).

Hernioplasty took longer than herniorrhaphy, at a mean of 45 minutes versus 38 minutes, the researchers note. Complication rates for seroma, haematoma, and wound infection were similar among both treatment groups, they found.

However, during a mean of 64 months of followup, the rate of hernia recurrence was significantly higher (p = 0.0015) among patients undergoing herniorrhaphy (11%) compared with those undergoing hernioplasty (1%), Dr. Arroyo's team says. The recurrence rate was not significantly affected by the size of the hernia, they add.

This low recurrence rate, which is similar to that achieved for prosthetic repair of inguinal hernia, means that "prosthetic repair could become the standard treatment for primary umbilical hernia in adults," Dr. Arroyo and colleagues conclude.
