Type 1 Diabetics Should Be Screened for Parietal Cell Antibodies

Type 1 diabetics with parietal cell antibodies (PCAs) are at increased risk for the development of autoimmune gastritis, iron deficiency, pernicious anemia, and (pre)malignant gastric lesions, according to a report in the January issue of Diabetes Care.

The investigators suggest a management strategy that includes regular screening for these entities. “Approximately 15-20% of type 1 diabetic patients exhibit PCAs targeting gastric H+/K+ATPase,” write Christophe E. M. De Block, MD, PhD, and colleagues from the University of Antwerp in Edegem, Belgium.

 “We examined whether iron deficiency anemia, pernicious anemia, and autoimmune gastritis, which may predispose to gastric tumors, were more frequent in PCA+ than in PCA patients.” The investigators performed gastric biopsies on 88 consecutively recruited type 1 diabetic patients, including 51 men and 37 women, of whom 47 were PCA+ and 41 were PCA. Mean age was 42 ± 13 years.
