Paediatric Asthma Hospitalisation Depends on Clinical Indicators, Patient History

Hospital admission for paediatric asthmatics is associated primarily with both clinical indicators seen in the emergency department and previous patient-history factors.

North American researchers with the Multicentre Airway Research Collaboration say there are four major patient-history factors: previous hospital admission for asthma, intubation, recent corticosteroid use and comorbidity.

 They add that peak expiratory flow rate (PEFR) is predictive of admission when it is measured, but note the measurement is not often taken in emergency departments. The investigators led by Charles Pollack, MD, of Pennsylvania Hospital in Philadelphia, Pennsylvania, United States, conducted a prospective cohort study of patients aged two to 17 years presenting with acute asthma at 44 emergency departments in the US and Canada. Participants underwent a structured interview in emergency and again two weeks later. Admission decisions were made at the discretion of the treating doctor.
