An extra dose of salmeterol to counter dyspnoea safely prolongs duration of action. Patients gain no additional peak forced expiratory volume-1 (FEV1), however.
Investigators in Naples, Italy, looked at the effects on respiratory function of doubled salmeterol dosage in 15 patients with chronic obstructive pulmonary disease. The patients were receiving regular treatment with conventional doses of salmeterol.
On two different days, they inhaled these doses and, after four hours, inhaled another equal dose or placebo. Lung function was controlled before first drug administration and for the next 12 hours. In those who took the extra salmeterol, a mean 0.174 litre peak increase in FEV1 from baseline was reached after four hours.
In the placebo group, a mean 0.141 litre peak increase in FEV1 from baseline was reached after five hours.
