Cough without expectoration in young adults who do not respond to a course of antibiotics, a positive tuberculin test, x-ray changes and other clinical factors, suggest tuberculosis in patients with a negative sputum smear, a multinational team of researchers report.
Only about half of the patients with a confirmed diagnosis of pulmonary tuberculosis have positive sputum smear, Dr. B. Bah from Conakry, Guinea writes in the July issue of The International Journal of Tuberculosis and Lung Disease. Because sputum culture, rapid diagnostic methods and even x-rays are not available in most parts of sub-Saharan Africa, clinical criteria to diagnose smear-negative tuberculosis are needed.
