NEW YORK (Reuters Health) – Although children with atopy appear to have a greater risk of severe asthma the relationship between the two conditions still remains hard to define, according to Australian investigators.
This is the case, lead researcher Dr. Anne-Louise Ponsonby told Reuters Health, "because asthma is not a uniform disease. Some children with asthma will have mainly house-dust mite allergic airway disease. Some children with asthma will have symptoms only triggered by a virus. Thus, we need to look at disease subgroups within the broad spectrum of asthma."
As part of such an effort, Dr. Ponsonby, of Australian National University, Australian Capital Territory, and colleagues examined data on a randomly selected population sample of 758 children, 8 to 10 years old, and another group of 78 children who had received hospital asthma treatment during a 1-year period.
"For children with a history of asthma ever in the community, a large proportion of asthma did not appear to be associated with allergy," Dr. Ponsonby continued. However, "among children with more than 12 wheeze episodes over the past year, a large proportion of this frequent wheeze could be attributed to allergy. This group of children is the group with more severe disease."
In fact, atopy was "moderately" related to ever having asthma (odds ratio, 2.09) but "strongly" related to hospital attendance (odds ratio, 16.95). These findings, the investigators conclude, demonstrate that disease severity should be considered "when examining the relationship between aeroallergen sensitization and childhood asthma."
In an accompanying editorial in the January issue of Chest, Dr. David N. Weissman, of Morgantown, West Virginia, notes that these results are in line with those of other studies, and agrees that it may "prove useful to define subpopulations based on severity of disease."
