Transtracheal open ventilation reduces inspiratory work in postextubation patients

WESTPORT, CT (Reuters Health) – Results of a recent study suggest that transtracheal open ventilation (TOV) effectively assists inspiration in postextubation patients with high upper airway resistance and improves their breathing patterns.

Dr. Akinori Uchiyama of Osaka University Hospital in Japan and colleagues conducted a prospective, controlled, crossover clinical study of 11 patients who had undergone mini-tracheotomy for sputum retention.

TOV was performed for a mean of 76.6 hours over 5.6 days. No major complications were observed, the research team reports in the September issue of Critical Care Medicine. Six patients had peak tracheal pressure measured distal to the mini-tracheotomy tube. Mean pressure was 4.33 cm H2O.

"Inspiratory tidal volume delivered by the ventilator was 0.51 L," according to the report. The respiratory rate was lower during TOV than during spontaneous breathing. The authors note that according to esophageal pressure and respiratory inductive plethysmography, the subjects' inspiratory work was reduced and their breathing pattern improved by TOV.

In addition, the researchers conducted a prospective laboratory lung model study comparing the effect of TOV with that of spontaneous breathing, open mini-tracheotomy, 5 L/min of transtracheal gas insufflation, and 5 cm and 10 cm H2O of pressure support ventilation (PSV).

The results showed that "mean tracheal pressure during TOV and 10 cm H2O PSV were positive values and they had larger inspiratory assistance according to the pressure-time product of pleural pressure." High upper airway resistance reduced the inspiratory assistance of PSV, according to the team, but the effects of TOV were unchanged.

"TOV may be useful for resolving some postextubation respiratory problems and forestall the necessity of reintubation," Dr. Uchiyama and colleagues conclude.
