WESTPORT, CT (Reuters Health) – Patients who stop smoking at least 4 weeks before undergoing pulmonary surgery appear to have fewer postoperative pulmonary complications compared with patients who continue to smoke, Japanese researchers report.
Dr. Masashi Nakagawa and colleagues from the Osaka Medical Center for Cancer and Cardiovascular Disease performed a retrospective study of 288 consecutive patients who underwent pulmonary surgery, according to their report in the September issue of Chest.
The researchers classified patients as current smokers (those who had smoked within 2 weeks of surgery), recent smokers (those who had not smoked 2 to 4 weeks before surgery), ex-smokers (those who had not smoked for more than 4 weeks before surgery), and never-smokers.
The incidence of postoperative pulmonary complications was significantly higher in current smokers (43.2%), recent smokers (53.8%) and ex-smokers (34.7%) compared with that in never-smokers (23.9%; p < 0.05), the researchers found.
For smokers who had stopped smoking 5 to 8 weeks before undergoing surgery, the incidence of postoperative pulmonary complications gradually decreased. For patients who had stopped smoking for 9 to 12 weeks before surgery, the incidence of postoperative pulmonary complications approached those of never-smokers, the investigators note.
To lower the risk of postoperative pulmonary complications, smoking cessation should occur at least 4 weeks before surgery, they conclude.
Dr. Nakagawa and colleagues also recommend "the development of an effective cessation program for outpatients to obtain the necessary length of smoking abstinence, and to make primary physicians aware of such programs."
