Difficulties Outlined In Studying Mood Disorders In The Very Elderly

A comparison of paroxetine and placebo use in 24 elderly, depressed residents of a nursing home illustrates the difficulties of studying mood disorders in the very elderly.

Researchers say they found no differences in improvement, using several outcome measures, between paroxetine and placebo groups of patients with a mean age of 87.9 years. But, a subgroup analysis of 15 patients indicated a trend toward greater improvement in the paroxetine group in one outcome measure.

 That measure combined the main outcome measure, the primary nurse’s Clinical Impression of Change, and interview-based measures. Patients in the subgroup also had higher Hamilton Depression Rating Scale and Cornell Scale for Depression scores at baseline.

Investigators from Boston University, Harvard Medical School, the Massachusetts Mental Health Centre and the Hebrew Rehabilitation Centre for Aged, all in Boston, and the University of Pittsburgh in Pennsylvania, United States, report two patients in the paroxetine group developed delirium.        Patients in the paroxetine group were also more likely to

experience a decrease in Mini Mental State Exam scores.
