Hyperactive children respond well to once-daily methylphenidate

WESTPORT, CT (Reuters Health) – A new once-daily methylphenidate formulation appears effective in treating children with attention-deficit/hyperactivity disorder (ADHD), researchers report in the October issue of Pediatrics.

In fact, lead author Dr. Mark L. Wolraich told Reuters Health that "the once-a-day, long-acting methylphenidate worked as well as the three-times-a-day, regular methylphenidate."

Dr. Wolraich of Vanderbilt University in Nashville, Tennessee and colleagues conducted a double-blind study of 282 children with ADHD. Of these, 90 were randomized to placebo, 97 to standard immediate-release methylphenidate three times daily, and 95 to extended-release methylphenidate once daily.

By the end of the first week, children in the methylphenidate groups showed significantly greater reduction in core ADHD symptoms than did those in the placebo group. This persisted for the remainder of the 4-week treatment period.

There were no significant differences in responses between the methylphenidate groups. Forty-eight percent of placebo patients stopped treatment prematurely versus 14% of immediate-release and 16% of extended-release methylphenidate patients.

The researchers thus conclude that for treatment of core ADHD symptoms, the two formulations "were not significantly different from each other."

Dr. Wolraich also pointed out that "the long-acting form has the distinct advantage of not requiring any doses to be given during the school day, so that the treatment can be provided without involving the school personnel in the dispensing of medication."
