Angiotensin-Converting Inhibition Fails To Ward Off Retinopathy In Normotensive Diabetics

Angiotensin-converting enzyme inhibition for moderate to severe diabetic retinopathy is unlikely to help normotensive type 2 diabetic patients.

 A pilot study was stopped prematurely after a mean duration of 7.2 months because small doses of enalapril did not retard the progression of moderate to severe diabetic retinopathy. Earlier reports had indicated that angiotensin-converting enzyme inhibitors might have a beneficial effect on mild diabetic retinopathy.

Specialists at the Departments of Medicine and Ophthalmology, Charles R. Drew University, Los Angeles, California, United States, undertook the study to see if enalapril 5.0 mg. would hold off progression of moderate to severe diabetic retinopathy in normotensive type 2 diabetics. The patients were selected to isolate the effect on the ocular renin-angiotensin system independent of any blood pressure lowering.
