Haemopoietic stem cell transplants can be curative for patients with advanced polycythemia vera (PV) or advanced essential thrombocythemia (ET).
United States researchers from three centres in Seattle, Washington, report three-year survival of 64 percent in 25 patients they studied at the Fred Hutchinson Cancer Centre. Twelve PV and 13 ET patients were followed for a median of 41 months. Follow-up ranged from five to 118 months.
The patients ranged in age from 18 to 60 years, median age was 43 years. Median interval from diagnosis to transplant was 168 months.
The investigators found three reasons for transplantation in most cases: myelofibrosis with splenomegaly and peripheral blood cytopenias or development of a myelodysplastic syndrome or acute myeloid leukaemia. All but five patients were treated with cytotoxic agents before transplant. Nine had had splenectomies .
