WESTPORT, CT (Reuters Health) – Pregnant women with abnormal uterine artery Doppler test results in the second trimester are candidates for daily low-dose aspirin to prevent preeclampsia, UK researchers report in the November issue of Obstetrics and Gynecology.
In comments to Reuters Health, lead researcher Dr. Aravinthan Coomarasamy of Birmingham Women's Hospital noted that "a large Cochrane review recently showed that aspirin had statistically significant benefit in preventing preeclampsia, although the clinical significance of the finding was questioned as large numbers of women had to be treated to benefit a few."
This may be because the Cochrane review did not identify high-risk subgroups of women who may benefit the most from low-dose aspirin, the authors note in the journal report. The Cochrane Library is a database of medical literature.
Dr. Coomarasamy's team conducted a meta-analysis of five randomized trials that compared the effectiveness of aspirin with that of placebo or no treatment in women with abnormal uterine artery Doppler.
"[Our] systematic review shows that aspirin confers substantial [statistically and clinically significant] benefit in preventing preeclampsia in women identified as high risk by an abnormal second-trimester uterine artery Doppler examination," Dr. Coomarasamy said.
In the pooled analysis, women treated with aspirin had an odds ratio for preeclampsia of 0.55 compared with those who were untreated or received placebo. Sixteen women needed to be treated to prevent one additional case of preeclampsia, the investigators say, "making the screening and therapy with aspirin also clinically significant."
"We believe that giving low-dose aspirin treatment to these high-risk women in obstetric units where second-trimester ultrasound is routine is reasonable and could be added with little additional cost," Dr. Coomarasamy told Reuters Health.
