Stroke mortality significantly higher for women than men

WESTPORT, CT (Reuters Health) – Even after adjusting for age and comorbidity, female stroke patients are more likely to die than male stroke patients, according to findings presented during the American Heart Association's 3rd Scientific Forum on Quality of Care and Outcomes Research in Cardiovascular Disease and Stroke in Washington, DC.

Dr. Allan L. Anderson of the Hospital at Medical City in Dallas and colleagues analyzed data on 74,964 patients (56% female) with a primary diagnosis of stroke treated at 195 hospitals.

Women in this study population were significantly older–78 years on average, compared with 73 years on average for men. The female stroke patients also had more comorbid conditions than male stroke patients.

The researchers conducted logistic regression analysis of adverse outcomes including mortality, shock or hemorrhage, and acute renal failure as well as cardiac, respiratory, or neurologic complications to determine gender differences in outcomes

"The most significant finding is the fact that, after risk adjustment, women do have a higher mortality than men do after stroke," Dr. Anderson told Reuters Health. . The adjusted odds ratio for mortality was 1.2 for women compared with men. "This finding is similar to that for other cardiovascular diseases, such as acute MI and cardiac procedures."

Conversely, male stroke patients were 1.4 times more likely to have acute renal failure than female stroke patients. Dr. Anderson added, "the significantly higher incidence [3.6 times] of cardiac complications in men was also noteworthy."

"The men seem to have been a 'sicker' population," he explained, "having higher incidences of prior MI, smoking, type 2 diabetes, and peripheral vascular disease [as well as] a significantly higher likelihood of prior coronary artery bypass surgery or percutaneous intervention."
