CT unnecessary in work-up of many patients with acute pancreatitis

WESTPORT, CT (Reuters Health) – Computed tomography is probably not needed in the initial work-up of mild cases of acute pancreatitis, and late CT followup is probably only necessary in cases with clinical or biologic deterioration.

While serial CTs have been advocated as a means of following patients with acute pancreatitis, few studies have actually examined the benefits of this monitoring strategy, Dr. Yves Panis and colleagues, from Hopital Lariboisiere in Paris, note in the August issue of the Journal of the American College of Surgeons.

In the current study, Dr. Panis' team assessed the role of CT in 102 patients admitted with acute pancreatitis. Contrast-enhanced CT was performed on admission and on hospital day 7.

Twenty-three percent of patients developed complications, the authors note. However, only 8% of patients with Ranson scores <2 developed complications, suggesting that admission CT was unnecessary in this subgroup.

Followup CT appeared to be useless for patients with Ranson scores 90% of the complications can be suspected only by clinical and biologic assessment, we believe that…late CT should be proposed only in selected high-risk patients with suspicion of clinical or biologic deterioration," the investigators state.
