ה-NKF האמריקני פרסם הנחיות חדשות לטיפול בחולי כליה , המסמך מתבסס על שתי עובדות עיקריות:
1 עליה ברמת הזרחן בדם מעלה את הסיכוי לתמותה בקרב חולי כליה, ויש להורידה אל מתחת לרמות המקובלות כיום
2 יש להגביל את כמות הסידן הנצרכת על ידי חולי כליה, בכדי למנוע הסתיידות בכלי הדם וסיבוכים נלווים
בשנים האחרונות התברר כי עליה ברמת הזרחן בדם מעלה את הסיכון לתמותה בקרב חולי כליות. איגוד הכליה הלאומי האמריקני (National Kidney Foundation) פרסם לאחרונה הנחיות חדשות בנושא הטיפול בחולים, המציבות יעדים מעודכנים בשאיפה לאיזון הזרחן והסידן במטופלי הדיאליזה. ההנחיות דנות בדרכים הטיפוליות המומלצות בכדי להגיע לאיזון טוב יותר, במטרה להפחית את התחלואה והתמותה בקרב חולי כליות.
בין היתר ממליצות ההנחיות החדשות להוריד את רמת הזרחן בדם אל מתחת לרמות המקובלות כיום, על ידי שימוש בתרופות קושרות זרחן ובתזונה מתאימה. כמו כן מומלץ להגביל את כמות הסידן שמקבלים החולים, כדי להפחית את תהליך הסתיידות כלי הדם, המאפיין חולי כליה.
בארץ מצויים כיום למעלה מ-4000 מטופלים בדיאליזה, וקיימת עליה ניכרת במספר חולי הכליות מדי שנה.
ה NKF האמריקני הנו גוף המאגד רופאים וחוקרים, אחיות, דיאטנים ומטופלי דיאליזה, המפרסם המלצות בנושאים הקשורים באיכות הטיפול בדיאליזה. הנחיות ה-NKF מגובשות לאחר מחקר מקיף ומשמשות כבסיס לגישה הטיפולית בארה”ב.

הנחיה מסעיף 3: Evaluation of serum phosphorus levels
In CKD patients with kidney failure (Stage 5) and those treated with hemodialysis or peritoneal dialysis, the serum levels of phosphorus should be maintained between 3.5 and 5.5 mg/dl (1.13 and 1.78 mmol/L). (Evidence)
הנחיות מסעיף 5:Use of phosphate binders in CKD
Both Calcium-based binders and other non calcium- ,non aluminum-, non magnesium containing phosphate agents (such as Sevelamer HCL) are effective in lowering serum phosphorus levels (Evidence) and either may be used as primary therapy.(Opinion)
The total dose of elemental calcium provided by the calcium based phosphate binders should not exceed 1500 mg/day (opinion), and the total intake of elemental calcium (including dietary calcium should not exceed 2000 mg/day.(Opinion)
Calcium based phosphate binders should not be used in dialysis patients who are hypercalcemic (corrected serum calcium of > 10.2 mg/dl [2.54 mmol/L]), or whose plasma PTH levels are < 150 pg/mL ( 16.2 pmol/L) on two consecutive measurements. (Evidence).
Noncalcium-containing phosphate binders are preferred in dialysis patients with severe vascular and/or other tissue calcification.
הנחיות מסעיף 6:Serum calcium and calcium-phosphorus product
In the event corrected total serum calcium level exceeds 10.2 mg/dL (2.54 mmol/L), therapies that cause serum calcium to rise should be adjusted as follows:
In patients taking calcium-based phosphate binders, the dose should be reduced or therapy switched to a noncalcium-, nonaluminium-, nonmagnesium-containing phosphate binder. (OPINION) See Guideline 5.
