Local anaesthesia delivered through a novel indwelling retrobulbar catheter produces durable and titratable pain control during and after pars plana vitrectomy.
Researchers from the University of Heidelberg, Mannheim, Germany, and the University of Montreal, Canada, treated 88 patients, aged between 37 and 88 years, undergoing pars plana vitrectomy with 7 mL of 2% mepivacaine. The anaesthetic was delivered using a novel retrobulbar catheter, which remained in place for 24 hours after the procedure. During and following the operation, patients rated their pain on a scale of 0 to 10. The surgeons injected a further 2 mL mepivacaine when the grade exceeded 3.
In 35% of procedures, surgeons administered the first intraoperative injection a mean of 53 minutes after the start of vitrectomy. In 12 and 4% of procedures, patients required a second and third injection respectively.
