Celecoxib Good Choice for Arthritic Patients With Ulcers

In a randomized trial reported in the Dec. 26 issue of the New England Journal of Medicine, recurrence of ulcers and adverse events in arthritis patients who were negative for Helicobacter pylori were the same or less with celecoxib therapy than with combination treatment with diclofenac and omeprazole.

 “Current guidelines recommend that patients at risk for ulcer disease who require treatment for arthritis receive nonsteroidal anti-inflammatory drugs (NSAIDs) that are selective for cyclooxygenase-2 (COX-2) or the combination of a nonselective NSAID with a proton-pump inhibitor,” write Francis K.L. Chan, MD, and colleagues from the Prince of Wales Hospital in Hong Kong, China.

 The intention-to-treat analysis in this study included 287 patients presenting with bleeding ulcers who had used NSAIDs for arthritis and who were negative for H. pylori. After their ulcers had healed, they were randomized to six months of treatment with celecoxib, 200 mg twice daily plus daily placebo, or 75 mg diclofenac twice daily plus 20 mg omeprazole daily.
