Implantable cardioverter defibrillators (ICD) with rate-responsive dual-chamber pacing offer no clinical advantage over back-up ventricular-only pacing, according to a new study released early from the Dec. 25 issue of The Journal of the American Medical Association. Researchers at the Cleveland Clinic stopped their study early after determining that patients with dual-chamber rate-responsive pacing and no other indication for cardiac pacing had increased rates of the combined end points of hospitalization for heart failure or death.
Bruce Wilkoff, MD, told Medscape that fewer than 20% of patients who need defibrillators for irregular heart rhythms also need pacemakers. ICDs sense abnormal heart rhythms and deliver an electric shock when such rhythms are determined to be dangerous. Dual-chamber pacemakers supply electrical impulses to both the upper and lower chambers of the heart, and can be programmed to either rate-responsive pacing or back-up only pacing.
