Hip replacement is needed less frequently in patients with osteonecrosis of the femoral head when core decompression and autologous bone marrow grafting is performed in Stage I or Stage II disease, before collapse. French researchers report hip replacement was needed in only nine of 145 hips in which patients were treated before collapse.
This compared to 25 of 44 hips in which patients were treated after collapse, in Stage III or Stage IV disease. Investigators from Hopital Henri Mondor in Creteil, France, say there are many techniques used in treating osteonecrosis of the femoral head through core compression with bone graft.
They did grafting with autologous bone marrow obtained from the iliac crest in 189 hips in 116 patients. Outcome was determined using changes in the Harris hip score, progression in radiographic stages and the need for hip replacement.
