Severe restriction of protein intake in patients with overt diabetic nephropathy does not appear to delay the advance of renal disease and could, in fact, lead to malnutrition, say a team of physicians in Rome, Italy, who investigated whether the progression of chronic renal failure might be delayed by restricting protein intake.
In this prospective clinical trial, renal function and nutritional status were evaluated in 69 nephrology outpatients with both overt diabetic nephropathy and hypertension. The 32 patients with type 1 diabetes and 37 with type 2, were treated with insulin. The study began when hypertension and glycemia had been controlled for at least three months.
Researchers matched the two groups for similar mean glomerular filtration rate value and nutritional status. Thirty-five patients were randomly prescribed a low protein diet (0.6 g/kg/d) and a free diet intake was maintained by the other 34 patients for a year.
