Chronic reactive arthritis does not respond to long-term doxycycline

WESTPORT, CT (Reuters Health) – Doxycycline does not appear to be an effective treatment for chronic reactive or undifferentiated seronegative arthritis, according to a research team from McMaster University in Hamilton, Canada.

In a study reported in the Annals of Rheumatic Diseases for December, 60 patients were randomized to a 90-day course of treatment with doxycycline 100 mg b.i.d. or placebo. The patients had had signs of acute joint inflammation for a median of 24 months and had negative laboratory tests for rheumatoid factor and antinuclear antibodies.

Sixteen patients assigned to doxycycline and 21 assigned to placebo completed 3 months of the study. Dr. M. Smieja and colleagues found that, at the end of treatment, there were no significant differences between groups for pain change scores, composite functional change scores, or rheumatologist-assessed arthritis activity.

For 16 patients who were again examined at 6 to 12 months, the differences between groups remained insignificant. Eleven patients whose serology status indicated previous infection with Chlamydia trachomatis were also unaffected by treatment group.

Dr. Smieja's group notes that these results are comparable to those observed when the effects of other antibiotics–ciprofloxacin or lymecycline–were evaluated in randomized trials involving similar populations of patients. However, the authors say they cannot rule out a beneficial treatment effect if doxycycline were initiated early in the course of C. trachomatis-associated reactive arthritis
