WESTPORT, CT (Reuters Health) – Indiscriminate use of antibiotics significantly increases the risk of delayed hospital admission and missed diagnosis of infectious and noninfectious diseases, according to a report in the October 22nd Archives of Internal Medicine.
"Inappropriate use of antimicrobial drugs is a serious medical and societal problem," contend Dr. Calvin M. Kunin from Ohio State University in Columbus, Ohio and colleagues.
The authors base that conclusion on the results of their study in a large general hospital in Taiwan, where they assessed antimicrobial activity in urine and time of hospital admission and diagnosis of 444 patients. Patients with infection and antimicrobial activity in urine were 1.61 times more likely to experience delayed admission and 1.91 times more likely to have a masked/missed diagnosis than were patients with infection and no antimicrobial activity in urine, the authors report.
Similarly, patients without infection but with antimicrobial activity in urine were 1.87 times more likely to have had a missed diagnosis than those without antimicrobial activity in urine, the results indicate.
"Physicians should prescribe antibiotics only for bacterial infections," Dr. Kunin told Reuters Health. "If a patient insists on obtaining an antibiotic despite obvious evidence that it is not needed, the physician should offer the patient the opportunity to seek medical care elsewhere," he suggested.
He also offered a number of other recommendations to avoid inappropriate antibiotic use.
"The pharmaceutical companies should stop conning physicians into believing that their newest and most expensive drugs can displace good medical evaluation and judgment," Dr. Kunin said. "Antibiotics should not be advertised on television or surreptitiously inserted into story lines in TV programs. Antibiotics should not be made available without prescription."
