NEW ORLEANS (Reuters Health) – Hormone replacement therapy (HRT) improves endothelial function in postmenopausal women at risk for cardiovascular disease but raloxifene does not achieve the same result, according to study findings presented here at the 12th annual meeting of the North American Menopause Society.
Senior investigator Dr. Guiseppe M. C. Rosano from San Raffaele in Rome and colleagues enrolled 20 postmenopausal women considered at increased risk of cardiovascular disease. In a double-blinded protocol, the women were randomized to either 0.625 mg conjugated equine estrogen and 2.5 mg medroxyprogesterone acetate (Prempro) or 60 mg raloxifene for 4 weeks, then crossed over to the other regimen.
Dr. Rosano was unable to attend the meeting, but Dr. Marius Jan van der Mooren of Amsterdam, who was not a co-investigator but who was familiar with the study, presented the findings. Dr. Mooren said that endothelium-dependent flow mediated vasodilation "increased from 7.4% to 12.4% with HRT but there was no increase evident with raloxifene."
Endothelin-1 decreased from 3.2 to 2.8 in women on HRT, but there was no change in those on raloxifene, Dr. Mooren said. "Nitrite and nitrate levels increased during Prempro, but not during treatment with raloxifene," he said.
Dr. Philip M. Sarrel of Yale University challenged the findings. Dr. Sarrel said he and associates conducted a similar study in which the results were exactly opposite: raloxifene was beneficial but HRT was not. He said, too, that the only published study suggests that both HRT and raloxifene are beneficial.
"What this tells us is that there is no conformity between the laboratories where these studies are done," Dr. Sarrel told Reuters Health. "We also don't know these tests and measurements are clinically significant
