WESTPORT, CT (Reuters Health) – High levels of glucose in nondiabetic pregnant women are associated with a greater risk of pregnancy complications, including cesarean section and clinical chorioamnionitis, study results suggest.
Dr. Theresa O. Scholl, of the University of Medicine and Dentistry of New Jersey, in Stratford, and colleagues examined the influence of maternal glucose concentration on the course and outcome of pregnancy in 1157 nondiabetic gravidas. They used 1990 to 1995 data from the Camden Study, a prospective study of young women residing in Camden, New Jersey.
"Controlling for potential confounding variables…and comparing women whose plasma glucose concentrations were <99 mg/dL with the others indicated that birth weights were approximately 50 g higher (p < 0.05) for gravidas whose glucose concentrations were 99 to 130 mg/dL and 200 g higher (p 130 mg/dL," the team reports.
Increased maternal glucose concentration also correlated with a greater risk of large-for-gestation fetuses (p < 0.001), the authors explain in the September 15th issue of the American Journal of Epidemiology.
Length of gestation was inversely associated with glucose level, being significantly shortened by 2 to 5 days when concentrations of glucose were high. Furthermore, high glucose concentrations resulted in a twofold increased risk of maternal complications, including cesarean section and clinical chorioamnionitis. Chorioamnionitis combined with a high maternal glucose concentration resulted in a nearly 12-fold increased risk of very preterm delivery.
These findings "raise questions about whether higher but seemingly normal maternal plasma glucose concentrations are associated with very preterm delivery by predisposing to or acting as a marker for placental inflammation and subclinical infection," Dr. Scholl and colleagues conclude.
