Renal insufficiency linked to risk of cardiac events in older women with CHD

WESTPORT, CT (Reuters Health) – Mild to moderate renal insufficiency appears to be an independent predictor of cardiac events in postmenopausal women with coronary heart disease (CHD), according to a report in the September issue of the Journal of the American College of Cardiology.

Dr. Michael G. Shlipak from the Veterans Affairs Medical Center, San Francisco, and colleagues collected data on 2763 postmenopausal women with CHD who participated in the Heart and Estrogen/progestin Replacement Study. The researchers categorized the women as having normal renal function or mild or moderate renal insufficiency.

The patients were followed for a mean of 4.1 years. Multivariate analysis showed that compared with women with normal renal function, women with mild or moderate renal insufficiency were at increased risk for death from CHD, nonfatal myocardial infarction, hospitalization for unstable angina, stroke and transient ischemic attacks. The relative hazard for these cardiovascular events was 1.24 with mild renal insufficiency and 1.57 for moderate renal insufficiency.

The independent association of moderate renal insufficiency with death from CHD and stroke and transient ischemic attacks was statistically significant, but the association with nonfatal myocardial infarction or unstable angina was not, the investigators note.

"This study supports the hypothesis that declining renal function, from mild reductions to end-stage renal disease, may steadily increase the risk for cardiovascular events," Dr. Shlipak and colleagues conclude.
