Skin symptoms ‘may mean patients have immune disorder’

Dermatologists have been asked to be on the alert for signs of interstitial granulomatous dermatitis. Patients found to have the condition on clinical examination may have an underlying systemic autoimmune disorder, researchers from the University of Turin have concluded after reviewing 17 patients (all but one women).


From the histological, serological and clinical findings the researchers emphasize that interstitial granulomatous dermatitis with plaques is a ‘distinct entity with highly reproducible clinical and histopathological features’.


The researchers report that most of the patients had rheumatoid polyarthralgias along with abnormalities in the serology findings – usually linked to collagen vascular disorders. On histology examination there was a distinctive dermatitis with the interstitium infiltrated diffusely with histiocytes.


There was also piecemeal fragmentation of collagen, and small granulomas were formed around the degenerative areas. The numbers of polymorphonuclear leukocytes within the infiltrate varied between the individuals.
