Early endotoxin exposure may protect against eczema

NEW YORK (Reuters Health) – Infants exposed to high concentrations of endotoxin appear to develop protection against atopic eczema during the first 6 months of life, but their risk of nonspecific respiratory disease is increased, German researchers report.

Dr. Michael Borte from the University of Leipzig, and colleagues collected data on 1884 infants. When the infants were 3 months old, the researchers collected samples of dust from the beds of the infants and their mothers.

At 6 months, infants who were exposed to the highest concentrations of endotoxin (fifth quintile) from their mothers' mattresses had a significantly decreased risk of atopic eczema (odds ratio 0.50). However, these infants had an increased risk of respiratory infection (odds ratio 1.69) and cough with respiratory infection, bronchitis or both (odds ratio 1.73), the researchers found.

The risk of developing wheeze was also significantly increased (odds ratio 2.37). By the end of the first year of life, except for the risk of wheeze (odds ratio 1.60), the risk of other respiratory problems attenuated, according to the report in the November issue of the Journal of Allergy and Clinical Immunology.

With the exception of cough with respiratory infection, bronchitis or both, the association between eczema, respiratory disease and endotoxin concentrations was not found in dust samples taken from the infants' mattresses, Dr. Borte's group points out.

Dr. Borte and colleagues conclude the findings "support the hygiene hypothesis that exposure to high concentrations of endotoxin very early in life might protect against the development of atopic eczema, along with an increased prevalence of nonspecific respiratory diseases."
