Topical nitroglycerin diminishes vaginal and labial pain

WESTPORT, CT (Reuters Health) – Topical nitroglycerin significantly decreased pain intensity in a small cohort of women with vulvodynia, according to data presented Saturday at the Female Sexual Function Forum in Boston.

"[This] may prove to be a promising therapy in women with vulvar and labial pain," Dr. Jennifer Berman, of the University of California Los Angeles Medical Center, told Reuters Health.

Dr. Berman and colleagues conducted an open-label study using 0.2% nitroglycerin cream for relief of vaginal and labial pain in 23 women. In the office setting the researchers found that all patients, who had a mean age of 32 years, experienced relief within 5 minutes of treatment.

Dr. Berman's team then instructed the patients to use about 1/8 teaspoon of the cream on an as-needed basis prior to sexual relations at home. The patients were told "to apply the ointment 15 minutes prior to [sexual] relations so that most of the medication would be absorbed," Dr. Berman said.

About 95% of the patients reported that application of the cream significantly decreased pain related to vaginal penetration. But about 95% also experienced headache, apparently due to the amount of cream applied, Dr. Berman said. The patients' partners noted no negative side effects.

Dr. Berman termed the early findings in this pilot study "promising." She noted that larger, placebo-controlled studies will be necessary to establish the optimum dosage and "to standardize the delivery method that will diminish headache symptoms."
