ACE Inhibitor Therapy May Be Harmful for Small Subgroup of MI Patients


Previous reports have shown ACE inhibitor therapy to be beneficial for patients who have just experienced an acute myocardial infarction. However, recent findings indicate that such therapy may increase the risk of death for patients with a history of hypertension but low systolic blood pressure at presentation.


Dr. Fausto Avanzini, from the Hospital of Desio in Italy, and colleagues assessed the outcomes of 18,023 patients who were randomized to start treatment with lisinopril or placebo within 24 hours of acute myocardial infarction onset. Of the patients studied, 10,661 had a history of normotension and 7362 had a history of hypertension.
