WESTPORT, CT (Reuters Health) – Heart failure patients treated with the loop diuretic torsemide seem to fare better than patients receiving furosemide, possibly because of the more predictable bioavailability of torsemide.
Dr. Michael D. Murray from Regenstrief Institute for Health Care in Indianapolis and colleagues randomized 234 patients with chronic heart failure to furosemide or torsemide, according to a report in the November issue of the American Journal of Medicine.
During the 1-year, open-label trial, patients receiving torsemide were significantly less likely to be readmitted to the hospital for heart failure compared with patients receiving furosemide (17% vs 32%, p < 0.01). Also, admissions for all cardiovascular causes were significantly lower (p = 0.03) among patients taking torsemide (44%) than patients taking furosemide (59%). However, there was no difference in hospital admission for all causes among patients receiving torsemide (71%) and those receiving furosemide (76%).
Dr. Murray's team notes that patients in the torsemide group had improved dyspnea and fatigue scores from baseline, but only the improvement in fatigue score was statistically significant.
"These observations are potentially of great importance and affect most of the 5 million patients in the US who have heart failure and who use diuretics," Dr. Jay N. Cohn from the University of Minnesota Medical School, Minneapolis, comments in a journal editorial.
Dr. Cohn points out that a double-blind placebo controlled study is needed to confirm these findings. "If a trial confirmed and extended the observations of Murray and associates, it might substantially influence the choice of diuretic for managing patients with heart failure, and with considerable economic benefit," Dr. Cohn writes.
