Team sports not necessarily off-limits for children with solitary kidney or testis

SAN FRANCISCO (Reuters Health) – Based on a retrospective review of 14,763 cases included in a trauma registry, physicians could consider easing restrictions on contact team sports for children with one kidney or a single testis.

The findings were presented at the section on urology at the American Academy of Pediatrics 2001 National Conference and Exhibition by Dr. Irene M. McAleer.

Dr. McAleer said that patients with a solitary testis or kidney are often advised to refrain from team contact sports to protect the organ, but that there are no data to support these recommendations. She and her colleagues at Children's Hospital and Health Center, San Diego, reviewed the hospital's trauma registry covering the period July 1984 to December 2000 to identify patients treated for renal of testis injury.

They identified 209 cases in which renal or testis injury was confirmed by imaging study or surgical evaluation. One hundred ninety-three were renal injuries and 16 were testis injuries. Patients ranged in age from 2 months to 17.5 years, she said. The majority, 152 or 72.7% were boys.

More than half of the injuries occurred during a recreational activity such as "biking or playing on monkey bars in a playground," Dr. McAleer said. Only 10 injuries, six of them renal, were sustained during team sports. Three of the renal injuries were attributed to football, one to soccer and two to basketball. "No renal injuries were associated with baseball," she said.

She said that children are often eager to participate in team sports and suggested that pediatricians should consider easing some restrictions. She said, however, that doing so would require careful counseling of both children and parents.

During the comment period, Dr. McAleer was asked if she would allow these children to participate in team sports. "All children have a solitary spleen and a solitary liver, but that has never been considered as a basis for restricting activity," Dr. McAleer pointed out.
