Parathyroidectomy may improve sexual function in men with hyperparathyroidism

NEW YORK (Reuters Health) – Among men with symptomatic hyperparathyroidism secondary to renal failure, parathyroidectomy and autotransplantation appears to restore libido and sexual function, according to a recent report by Taiwanese investigators.

Chronic renal failure patients often experience disturbances in sexual function. It has been suggested that elevated parathyroid hormone (PTH) levels found in these patients may disturb other hormones necessary for normal sexual function. In the current study, Dr. Fong-Fu Chou from Chang Gung Memorial Hospital in Kaohsiung and colleagues investigated whether reducing PTH levels, through surgical means, would improve sexual function in male patients with symptomatic secondary hyperparathyroidism.

The study included 20 men who underwent total parathyroidectomy with autotransplantation of 90 mg of tissue into the forearm or thigh, the researchers report in the November issue of the Journal of the American College of Surgeons.

Three months after surgery, serum levels of calcium, phosphorus, alkaline phosphatase, follicular stimulating hormone (FSH), intact PTH, and prolactin were significantly lower than preoperative levels. Despite the change, FSH levels remained within normal limits and 70% of alkaline phosphatase levels were still above normal.

Based on International Index of Erectile Function scores, sexual function improved significantly after surgery. In addition, the monthly frequency of attempted intercourse increased and men reported more satisfaction with attempted intercourse and greater enjoyment with successful intercourse.

Dr. Chou's team believes that "the improvement of sexual function after parathyroidectomy and autotransplantation might be related to the reduction of prolactin, in association with the reduction of calcium, phosphorus, and intact PTH."
