Secondary osteoporosis prevalent in vertebral fracture patients in community

WESTPORT, CT (Reuters Health) – In a clinical setting, the characteristics of patients with osteoporotic vertebral fracture differ substantially from those of participants in epidemiological studies, according to researchers in Spain.

"Information about osteoporotic vertebral fracture has mainly been obtained via inferences from epidemiological studies of vertebral deformity," Dr. Joan M. Nolla and colleagues from the Universitאria de Bellvitge, in Barcelona, comment in the October issue of The Journal of Rheumatology.

They present what they say is the first clinical series of patients with osteoporotic vertebral fractures diagnosed in a rheumatological department. The 669 patients were examined over a 10-year period in a teaching hospital or at a primary healthcare center. Most, 80%, were women. The mean age at diagnosis was 67.1 years (range, 30 to 91 years).

The team reports that 177 patients (26%) were diagnosed with secondary osteoporosis, including 103 women and 74 men. The frequency of the diagnosis was significantly higher in men than in women, at 55% and 19%, respectively (p < 0.001). Long-term use of oral corticosteroids was associated with more than half of the cases of secondary osteoporosis.

Fifty-one percent of the patients presented with multiple fractures. "The prevalence of multiple fractures was higher in patients with secondary osteoporosis than in those with primary osteoporosis (66% versus 45%; p < 0.001)," the authors determined. "In corticosteroid-treated patients, the prevalence of multiple fractures was 68% and in patients with excessive alcohol consumption it was 71%."

Overall, 18% of the patients were admitted to the hospital. The frequency of admissions was higher in men than in women, at 27% and 16%, respectively (p < 0.01).

"In a rheumatological setting, frequency of secondary osteoporosis, mainly in relation to corticosteroid treatment, is a frequent condition and hospital admission is not a rare event," the research team concludes. "More studies in a clinical setting are needed to confirm our data."
