The increased 5-HT availability in panic disorder is important in maintaining the response to selective serotonin reuptake inhibitors.
The rapid lowering of brain serotonin function can precipitate panic symptoms in patients with paroxetine-responsive panic disorder when they are infused with the benzodiazepine antagonist flumazenil. Dr Caroline Bell and colleagues of the Psychopharmacology Unit, School of Medical Sciences, University of Bristol, England, investigated the effect of tryptophan depletion on 14 patients whose panic disorder had been successfully treated with the selective serotonin reuptake inhibitor paroxetine.
The patients received a tryptophan-free amino-acid drink on one occasion and a control drink on another in this double-blind crossover trial. They also received an infusion of flumazenil, as a pharmacological challenge and placebo on each day.
