Nonoperative management deemed best option for most midclavicular fractures

WESTPORT, CT (Reuters Health) – Nonoperative management of uncomplicated midclavicular fractures is associated with fewer complications than open reduction and intramedullary fixation (ORIF), and it allows patients to return to daily activities sooner.

That assessment comes from Dr. Frederico A. Grassi and colleagues, at the Universita dell'Insubria in Varese, Italy, based on their experience with 80 patients with uncomplicated midclavicular fractures. Half the patients were managed nonoperatively with figure-of-eight bandaging and half underwent ORIF with a 2.5-mm threaded pin.

As reported in the June issue of The Journal of Trauma, several complications occurred in the ORIF group, including eight superficial infections, three refractures, two delayed unions with pin breakage, and two nonunions. On average, patients in the nonoperative group returned to daily activities after 16.7 days, significantly sooner than ORIF patients at 40.7 days. Similar numbers of patients in each group reported complete treatment satisfaction.

"Noncomplicated fractures of the middle third of the clavicle can be successfully treated with either nonoperative or surgical methods," the researchers conclude. "Considering risks and costs, fixation procedures should be reserved for selected cases, in which nonoperative treatment does not guarantee sufficient chances of success, such as in the presence of severe displacement of the fracture fragments."
