Combination salmeterol and theophylline treatment benefits COPD patients

WESTPORT, CT (Reuters Health) – The combination of salmeterol plus theophylline appears to significantly improve lung function and symptoms in patients with COPD compared with treatment with either drug alone, researchers report.

During a 12-week trial, Dr. Richard L. ZuWallack from the St. Francis Hospital and Medical Center in Hartford, Connecticut, and colleagues randomly assigned 943 patients with COPD to salmeterol 42 µg b.i.d. plus theophylline, or salmeterol 42 µg b.i.d. or theophylline alone, according to their report in the June issue of Chest.

The researchers report that during the trial patients in all three treatment groups showed improvement from baseline.

However, patients who received salmeterol plus theophylline had significantly greater improvements in pulmonary function and significant reductions in dyspnea and albuterol use compared with either monotherapy group. In addition, patients who received salmeterol plus theophylline had significantly fewer COPD exacerbations.

Salmeterol monotherapy resulted in greater improvement in lung function and satisfaction, and was associated with fewer adverse side effects compared with theophylline alone or in combination with salmeterol, Dr. ZuWallack's group notes.

"The findings of Dr. ZuWallack and colleagues indicate that it is worth trying to achieve even a small improvement in lung function in COPD using combination bronchodilator therapy," Dr. Brian J. Lipworth from the University of Dundee in the UK comments in a journal editorial. This approach may lead to "meaningful improvements in disease control, quality of life, and exacerbations, even in some patients with apparent nonreversibility to albuterol."
