Lisinopril but Not Losartan Improves Myocardial Perfusion in Hypertensive Heart Disease

Long-term treatment with the ACE inhibitor lisinopril improves myocardial perfusion reserve (MPR) and maximal coronary flow in patients with hypertension-induced left ventricular hypertrophy. However, this effect is not seen with the angiotensin receptor blocker losartan, according to a report in the August 21st issue of the Journal of the American College of Cardiology.

“Studies have suggested that treatment with angiotensin-converting enzyme inhibitors improves MPR in patients with hypertension by potentiating endogenous bradykinins,” Dr. Olakunle O. Akinboboye, of St. Francis Hospital, Roslyn, New York, and colleagues note. “Because angiotensin receptor blockers lack a direct effect on bradykinins, we hypothesized that they may not improve MPR.”
